Participants fill out a form providing medical information in case of an emergency where the person cannot speak and/or the family member present is too distraught to assist in answering time/lifesaving questions. This form will provide the participant’s name, date of birth, present and past medical conditions, medications with dosage and frequency, emergency contacts and a primary doctor number, DNR (if applicable), or other important medical information. The form is stored in a clear plastic bag on the refrigerator. A red medical decal is placed on the bag and a second decal is placed on the front door of the home.
